HOPE Center was established in 1995 and over the past three decades we have strived to meet the primary care needs of our community. We want to partner with you to advance your health and wellness. Through our specialized programs, we can assist with connecting you to resources in the community that could support you in achieving your health goals.

HOPE Center is Your Partner in Health Care.

Our goal is to deliver comprehensive, compassionate, fair, and equitable medical care to all our patients. HOPE Center staff are multi-cultural and bilingual (English/Spanish) with trained and certified medical translators. HOPE Center has been a recognized Patient Centered Medical Home (PCMH) since 2017, offering comprehensive primary care and pain management services.

In addition to Primary Care, our medical doctors and Nurse Practitioners also specialize in delivering HIV primary care, Hepatitis C care and treatment along with being prescribers of PrEP (for HIV prevention).

HOPE Center receives specialized grant funding through NYS and NYC Department of Health (DOH) and Health Resources and Services Administration (HRSA). The funding allows HOPE Center to offer specialized medical and support services to individuals living with HIV and Hepatitis C, along with those seeking HIV preventive care through PrEP. HOPE Center also receives funding to provide community-based HIV/Hep C rapid testing and counseling services as well as linkage to care services. HOPE Center also has a NYS Medicaid funded Health Homes Care Management program which offers community-based Care Management services to qualifying Medicaid recipients. Health Homes Care ManagementSe habla Espanol
Our Health Homes Care Managers are available to individuals who reside in Westchester County, have Medicaid and are suffering from chronic conditions including Diabetes, High Blood Pressure, High Cholesterol, Mental Health, HIV, etc. Our skilled Medical Care Managers are available to assist clients with navigating the complex medical care systems including primary and specialty care appointments. They are also available to assist clients with connecting to community based social services such as Department of Social Services, local Food Pantries, Social Security, legal services, and more. Rapid HIV/Hepatitis C Counseling, Testing & ReferralSe habla Espanol
Our Prevention Program offers free and confidential rapid HIV and Hepatitis C (Hep C) testing in the community to individuals who do not know their HIV/Hep C status. These test results can be provided within 20 minutes. In addition, our team can assist with connecting you to preventive services such as PrEP/PEP and HIV or Hep C medical care and/or other community resources. Prevention Services for Our Community PartnersSe habla Espanol
Our Prevention team collaborates with many community partners by providing HIV/Hep C education and prevention tools and free testing at their sites.
